Referrals from Sydney support coordinators, families and health teams
We accept referrals from support coordinators, case managers, hospital discharge and allied-health teams, families and self-referring participants right across Sydney and NSW.
Referrals can be for NDIS home support, community access, supported independent living, respite, aged care support at home, Carer Gateway services or SSRC placements. Tell us what's needed and we'll confirm capacity honestly.
Every referral is reviewed within 24–48 hours by a local coordinator.
